Kansas City Chiefs WR Xavier Worthy exited Friday night’s season opener in São Paulo after just three plays following a collision with Travis Kelce, suffering a shoulder dislocation. Current reports indicate he’ll try to rehab and wear a shoulder brace this season while further testing is completed. Shoulder dislocations are typically managed either non-operatively (reduction + rehab) or surgically (stabilization), and the return-to-play timeline differs significantly. Below, I break down the injury update, treatment options, return timeline, and fantasy impact for Worthy.

Shoulder Dislocation Treatment Option 1: Non-Operative

We saw the training staff reducing Worthy’s shoulder (putting it back in the socket) on the sidelines, which is step one. Now, Worthy will have to rehab intensely to regain the stability in the shoulder. A shoulder dislocation almost always leads to tearing of the labrum. The labrum’s job is to passively hold the arm bone in the shoulder socket. When there is a labrum injury, this passive stability is impaired to varying degrees based on how much injury took place. When passive stability is impaired, the rotator cuff muscles must compensate and create “active stability” within the shoulder joint.

Increasing the active stability of the rotator cuff muscles is possible, and many NFL players have been able to return to finish out the season after a shoulder dislocation and undergo surgery in the offseason. In the NFL, the return timeline averages three games. If Worthy follows this trajectory, that would put him on track to return in week 5 against Jacksonville.

From a fantasy impact standpoint, rehab and a brace are the shorter path back if testing supports it. If he can return this season, his fantasy production should not be affected; however, NFL players have a 55% chance of re-dislocating, which could potentially lead to surgery. This is an incredibly high rate of re-injury and will make fantasy managers nervous every time they start him, but it is the better short-term option.

Shoulder Dislocation Treatment Option 2: Surgery

If further testing reveals a massive labrum tear and possibly more tissue damage, or if Worthy’s attempts at rehabbing fail, he will need surgery. Surgery of this type is followed by 5-6 months of rehab, which would end his season, but outcomes are excellent.

Numerous players have undergone surgeries to stabilize shoulder dislocations with no ill effects in the following season. Tee Higgins, Dalvin Cook, Baker Mayfield, Trevor Lawrence, just to name a few. Ultimately, Worthy will be able to return to 100% by next season, assuming he undergoes surgery at some point (now or after the season).

Fantasy Impact

Clearly, there are two distinctly different outcomes for Xavier Worthy this season. If he is able to return this year, he should play well, but the re-dislocation risk is incredibly high. For now, fantasy managers should hold onto Worthy in re-draft until more information is available. In dynasty leagues, if you are in win-now mode, continue to hold. If you are in a rebuild, consider trading an older player for Worthy.
